They represent an extremely heterogenous group, considered as the \u201cwaste basket\u201d of T-cell lymphomas. Their normal counterpart is a mature activated T-cell. Recently, progress has been made in regard to their biology resulting in the identification of different biologic subgroups and biologic prognostic factors and possible therapeutic targets, such as PDGFR-\u03b1. Patients with PTCL-NOS usually present with advanced disease and carry a poor prognosis. The International Prognostic Index (IPI), the Prognostic Index for T-cell lymphomas (PIT), as well as various biologic parameters, such as Ki-67 have prognostic significance. The optimal treatment strategy remains to be defined. The most frequently applied treatment is anthracycline-containing chemotherapy like CHOP. However, this approach can cure 30% of the patients approximately. CHOEP or CHOP-14 might be better candidates for firstline treatment. Young high-risk patients who achieve a complete remission can be consolidated with high dose chemotherapy and autologous stem cell transplantation (HDT\/ASCT). One third of relapsed\/refractory patients can be salvaged with HDT\/ASCT, if they are chemosensitive to second line chemotherapy. The remaining ones should be preferentially treated within clinical trials, testing new drugs or combinations, such as gemcitabine, Alemtuzumab, pralatrexate, depsipeptide or denileukin diftitox.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Haema 2010; 1(1): 42-51 by M.\u039a.
