It is estimated that 15\u201320 million people live with HTLV-I infection worldwide, but only 1-4% develop ATLL during a 70-year lifespan. High prevalence rates are found in Japan, parts of West Africa, Caribbean Islands, and South America. The low incidence and long latency of ATLL (more than 30 years) suggest that, in addition to viral infection, the accumulation of other genetic mutations may be needed for the development of ATLL. The diagnosis requires detection of ATLL cells (so-called flower cells) in blood, in patients with acute, chronic, or smoldering type with leukemic manifestations. ATLL is characterized by the rapid and uncontrolled clonal proliferation of mature transformed CD25+CD4+CD7- lymphocytes. Stools from all individuals, carriers and patients, should be screened for Strongyloides parasite even in the absence of symptoms. \u03a4he prevention of the disease by preventing HTLV-I infection is mandatory. Screening of blood donors for HTLV-I and II has become recently obligatory in our country even if the seroprevalence is expected to be low. It is obvious that a search for new therapies to confront this disease is needed. \u0396idovudine should be officially indicated and available for all ATLL patients around the world until novel treatments are generated. The role of other agents such as bortezomib, arsenic trioxide, and alemtuzumab remains to be further defined. No specific therapeutic recommendations can be made for smoldering ATLL or chronic ATLL at this time. The leukemic and lymphomatous forms of ATLL are best treated with interferon-\u03b1 and zidovudine with or without conditioning with chemotherapy. Investigation for HTLV-I antibodies should be mandatory in all patients with T-cell lymphoproliferative disorders.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Haema 2010; 1(1): 65-71 S.
